Animal control death of innocent dog that was already claimed

This year's Best City Screw-up ended in tragedy when Cachita, a small apricot poodle owned by Maria Stathos, was accidentally euthanized by Animal Control. On Feb. 4, 2005, the little dog briefly got loose and was picked up in front of her home by an Animal Control worker. Stathos went to claim her pet at Animal Control, paid $72 for her and was told to come back the next day to pick her up. When she returned, workers at Animal Control could not find her dog. Stathos was finally informed it had been mistaken for another poodle that was scheduled to be put down, and had been accidentally killed. We'd call that a pretty serious screw-up.
